Do Compression Socks Actually Do Anything? An Honest Look

A straight answer to the question everyone asks: what compression socks are actually proven to do, what they are not, and why the people they help most are often the last to notice.

It's a fair question, and it deserves a straight answer rather than a sales pitch. Here's what compression socks are actually proven to do, what they're not, and — the part almost nobody explains — why the people they help most are usually the last to notice.

The short version

Yes, for a specific job: reducing the swelling and heaviness that build up in your legs over a long day of standing or sitting still. That's measurable, it's been tested in randomised trials, and the mechanism is settled physiology rather than marketing.

No, for a lot of the things they get sold on. They won't sharpen your focus, boost your energy, or make you healthier in some general way. We'll come back to that, because being clear about the limits is the only way the real claims mean anything.

What's actually going on in your legs

Blood has to get back up your legs to your heart, against gravity. The pump that does it is your calf muscle — and it only works when it moves.

Stand still behind a counter for eight hours, or sit at a desk for eight hours, and that pump goes quiet. Blood pools in your lower legs. Pressure rises in the veins. Fluid pushes out into the surrounding tissue. By evening you have measurably more fluid in your lower legs than you did at breakfast.

That's the heaviness at hour ten. The sock groove still pressed into your ankle at bedtime. Shoes that fit at 8am and pinch at 6pm.

Graduated compression is firmest at the ankle and eases as it goes up the calf. That gradient gives the blood a direction — up — instead of letting it sit. As Harvard Health puts it: "By squeezing your legs, compression socks help the valves in your blood vessels do their job." Mayo Clinic describes the same thing: stockings that "gently squeeze your legs in a way that helps promote blood flow from the legs back toward the heart."

What the trials found

Swelling — the strongest evidence. One study measured leg volume at 7:00am and again at 7:00pm across three working days, in people sitting, standing, and mixed. Compression significantly reduced the fluid that accumulated over the day, in every position. Another randomised trial followed workers through 12-hour standing shifts: the group in regular socks developed measurable leg fatigue, swelling and discomfort by the end of the shift. The group in graduated compression didn't show those changes. Notably, the researchers found the lower compression level worked about as well as the higher one — you don't need to be squeezed harder to get the benefit.

Heaviness and aching — strong, with one honest caveat. A trial of 108 hairdressers at 15–20 mmHg found reduced pain and heaviness. What's interesting is that the symptom relief showed up even when the swelling hadn't fully dropped — people felt better before a tape measure would have agreed. The caveat: you can't blind someone to whether they're wearing compression socks, so some of that improvement could be expectation. That's why we say "may help" rather than "will fix."

Flights — real, but narrower than the internet claims. A Cochrane review of 12 trials and 2,918 travellers found graduated stockings sharply reduce symptomless DVT on flights over four hours. Here's where honesty matters: those studies recorded no symptomatic clots, no pulmonary embolisms, and no deaths in either group — so the research simply can't say stockings prevent serious clinical events. Any brand telling you their socks "prevent DVT" is going further than the evidence does.

What they don't do

We went looking for evidence that compression improves focus, alertness or mental clarity, because plenty of brands imply it. There isn't any. The one study that measured mood while wearing compression found no meaningful difference.

If your legs feeling better makes your evening better — and for a lot of people it genuinely does — that's a real experience. It isn't a proven medical effect, and we're not going to dress it up as one.

They're also not for everyone. If you have peripheral artery disease, diabetes with nerve involvement, neuropathy or reduced sensation in your feet, talk to your doctor before wearing compression. If you're pregnant, check with your OB first.

The reason people think they don't work

The better compression socks work, the less you notice them working.

Think about what the benefit actually is. It isn't a sensation that shows up. It's a sensation that stops showing up. The heaviness at hour nine doesn't arrive. The evening throbbing isn't there. The groove in your ankle at bedtime is gone.

Absence is almost impossible to notice. Your brain is built to flag new problems, not to send you a notification saying "that thing that used to hurt didn't happen today." Within two or three weeks, comfortable legs at 8pm have quietly become your normal — and you've forgotten what you were comparing them against.

These are moments that are hard to catch, easy to remember, and impossible to live through twice. Which is exactly why "I don't think they did anything" is such a common verdict from people they were, in fact, working for. They're not wrong about how they feel. They're wrong about the baseline, because the baseline moved underneath them.

How to test it on yourself properly

Two steps, and they cost nothing.

1. Write down day one. Before you put them on the first time, jot two or three lines somewhere: how heavy your legs feel, what your ankles look like, how you feel about the evening ahead. Read it again on day 21. Nobody can recall their own baseline three weeks later — this is the only honest comparison available to you.

2. Skip a day. Wear them daily for two to three weeks, then pick a normal day and don't. That's when the difference becomes obvious. Most people reach for the socks by lunchtime.

When they genuinely don't work

Three situations, and all three are fixable.

You put them on in the afternoon. This is the big one. Overnight, lying flat, your body reabsorbs the fluid in your legs — so at 7am your legs are at their smallest. A sock put on then fits as designed. The same sock at 5pm is being asked to squeeze a leg that's already swollen: hard to get on, uncomfortable, and half the benefit already lost. Morning, every time.

You guessed the size from your shoe size. A knee-high sock lives on your calf, not your foot. Two people wearing the same shoe size can be four inches apart in calf girth — and four inches is the difference between "supportive" and "cutting off my leg." Measure the widest part of your calf, in the morning, standing.

They're worn out and you can't tell. Elastane fatigue is invisible. The sock looks fine long after it has stopped applying meaningful pressure. The tell is simple: if they're noticeably easier to pull on than when they were new, they've lost their compression. Replace every three to six months of regular wear.

So — are they worth it?

If you spend eight to twelve hours a day standing still or sitting still, and your legs tell you about it by evening: the evidence says yes, and it's decent evidence. If you're active all day and your legs feel fine at night, you probably don't need them, and we'd rather say that than take your money.

The honest test is three weeks of actually wearing them, not three days of wondering.
